Comments
When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.
An anonymized string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy/. After approval of your comment, your profile picture is visible to the public in the context of your comment.
Media
If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.

HEALTHCARE PRIVACY
Ryde Chiropractic Pty Ltd is committed to providing quality health care for every client. As a fundamental part of this commitment, the Directors and their team recognise the importance of ensuring that our clients are fully informed and involved in their health care.
Ryde Chiropractic is a NSW health provider in the private sector, bound by the Health Records and Information Privacy Act (2002) and the Privacy Act 1988 (Cth) this includes both the Australian Privacy Principles and the NSW Health Privacy Principles. These principles set the standards by which we handle personal information collected from our patients. A copy of these Principles may be found at www.oaic.gov.au and www.ipc.nsw.gov.au.
As a part of our commitment to providing quality health care it is necessary for us to maintain files pertaining to your health. These files contain the following types of information:
Personal details (your name, address, date of birth, contact details);
Your medical history;
Notes made during the course of your chiropractic consultations;
Referrals to other health service providers;
Results and reports received from other health service providers.
The information held about you is provided by you or arises as a consequence of information provided by you.
Your medical file is handled with utmost respect for your privacy. The file will be accessed by your health care practitioner, and when necessary, for example in the absence of your usual health care practitioner, by other health care practitioners in the practice. It may also be necessary for our staff to handle your file from time to time to address the administrative requirements of running a health care practice. Our staff members are bound by strict confidentiality requirements as a condition of their employment and these requirements will be observed if it is necessary for them to view your records.
At times, to ensure the function of our practice, it may also be necessary to allow external organisations to access our practice and possibly, to view the medical records. Any external organisation that provides service or advice to this practice will be aware of the need to preserve the requirement of state and federal privacy legislation and will be bound by a confidentiality agreement.
The practice does not intend to disclose your personal information to overseas recipients.
We may use your information to send communication such as appointment reminders, email newsletters and other communication. You may choose to opt our of all or part of this communication by letting a staff member know.
Ordinarily we will not release the contents of your health care file without your consent. However, we advise that there may be occasions where we will be required to release the details of your file irrespective of whether you consent to the disclosure of the information is given.
We advise that as a patient of this practice you have rights of access to any information we hold concerning you. Should you wish to access this information we will ask you to complete a standard form to access this information in accordance with the Health Records and Information Privacy Act (2002) and the Privacy Act 1988 (Cth).
As part of our commitment to preserving the confidentiality of the information contained in your medical record we advise that strict storage policies are observed in this practice. All reasonable steps are taken to prevent any unlawful interference with your electronic records, which are accessible only by staff of this practice and are protected by security password. Any paper records are uploaded to your digital account and the paper copy is securely destroyed in house. Each member of staff is well versed in the principles and importance of doctor-patient confidentiality.
Should you, at any time, have a query or complaint in relation to the privacy policies in place at this practice please contact Dr Anthony Leong (Chiropractor), who will be happy to address any concerns you may have. We advise that this is the practice’s policy that any complaint is required to be made in writing and addressed to Dr Anthony Leong and marked private and confidential. We advise that we will make our best endeavour to address complaints within 30 days of receipt of your complaint.
Should you be unsatisfied with our response to your privacy complaint, you may lodge a written complaint with the NSW Privacy Commissioner or the Office of the Australian Information Commissioner.

CONSENT TO CHIROPRACTIC
Chiropractic care is recognised as being an effective and safe method of care for many conditions.
However, as in all health care, there are some very slight risks with chiropractic care. This includes, but is not limited to:
- Your condition becoming worse;
- Disc injuries, rib fracture, sprains/strains (1 in 139,000 in the neck and 1 in 62,000 in the low back) (1);
- Stroke or stroke like symptoms (1 in 5.85 million neck adjustments) (2).
Put in context, chiropractic has been shown to be 250 times safer than anti-inflammatory drugs (4) and safer than driving a car (5).
Some people may experience some mild soreness for 24 – 48 hours after their adjustments. This is a normal sign of change, as may occur after exercise or stretching.

AGREEMENT
I have read and understand the information above and discussed it with my chiropractor to my satisfaction.
I do not expect the chiropractor to be able to anticipate or explain all the risks and complications. I wish to rely on the chiropractor to exercise his/her judgment during the course of procedures which he/she feels, at the time, based upon the facts known, is in my best interests.
I have, to the best of my knowledge, provided the chiropractor with a complete and accurate health history. I have had the opportunity to discuss with the chiropractor the nature and purpose of chiropractic adjustments and other procedures as well as other concerns. I understand that results are not guaranteed. I intend this consent form to cover the entire course of my chiropractic care for this and any future presentation.
I hereby request and consent to chiropractic examinations, adjustments and other chiropractic procedures wherever the chiropractor determines necessary.

TERMS AND CONDITIONS
Cancellation Policy
Minimum 24 hours notice of cancellation required. Notification for instance, in person, via email, mobile phone ‘text message’ or any other means will be accepted subject to confirmation in writing. We reserve the right to levy a $30 charge to cover any subsequent administrative expenses.
Termination of Agreements and Refunds Policy
Both the Client and ourselves have the right to terminate any Services Agreement for any reason, including the ending of services that are already underway. No refunds shall be offered, where a Service is deemed to have begun and is, for all intents and purposes, underway. Any monies that have been paid to us which constitute payment in respect of the provision of unused Services, shall be refunded.
We aim to do our best to help you with your health concerns and provide a professional, high quality experience. At the same time, there is no guarantee of results.
Payment for services is due in full at the time of service.
